You kind of have to feel sorry for the kind of people who derive pleasure from other people's pain because in a way they have been conditioned by their environment to behave that way and can't really help it. IMO it hints at a certain amount of pain and self loathing within themselves. I think of it as a kind of hell to experience life through that mindset.
